Since the initial publication of A Compendium of Strategies to Prevent Healthcare-Associated Infections in Acute Care Hospitals in 2008, the prevention of healthcare-associated infections (HAIs) has continued to be a national priority. Progress in healthcare epidemiology, infection prevention, antimicrobial stewardship, and implementation science research has led to improvements in our understanding of effective strategies for HAI prevention. Despite these advances, HAIs continue to affect ∼1 of every 31 hospitalized patients,1 leading to substantial morbidity, mortality, and excess healthcare expenditures,1 and persistent gaps remain between what is recommended and what is practiced.
The widespread impact of the coronavirus disease 2019 (COVID-19) pandemic on HAI outcomes2 in acute-care hospitals has further highlighted the essential role of infection prevention programs and the critical importance of prioritizing efforts that can be sustained even in the face of resource requirements from COVID-19 and future infectious diseases crises.3
The Compendium: 2022 Updates document provides acute-care hospitals with up-to-date, practical expert guidance to assist in prioritizing and implementing HAI prevention efforts. It is the product of a highly collaborative effort led by the Society for Healthcare Epidemiology of America (SHEA), the Infectious Disease Society of America (IDSA), the Association for Professionals in Infection Control and Epidemiology (APIC), the American Hospital Association (AHA), and The Joint Commission, with major contributions from representatives of organizations and societies with content expertise, including the Centers for Disease Control and Prevention (CDC), the Pediatric Infectious Disease Society (PIDS), the Society for Critical Care Medicine (SCCM), the Society for Hospital Medicine (SHM), the Surgical Infection Society (SIS), and others.

Case of Candida auris Identified From the External Ear Canal of a Healthy Minnesota Outpatient With Travel to South Korea

Background:Candida auris is a globally emerging, multidrug-resistant fungal pathogen that causes healthcare-associated outbreaks and can be misidentified in clinical laboratories. Most US C. auris cases occur in hospitalized or long-term care patients with underlying medical conditions. Also, 4 global phylogenetic C. auris clades largely cluster geographically. Receiving health care abroad is a risk factor for US C. auris cases. In December 2019, the Minnesota Department of Health (MDH) confirmed Minnesota’s first C. auris case, isolated from the external ear canal of a healthy young adult outpatient with right-sided otitis externa. We describe the investigation and response for this uncommon US presentation of C. auris. Methods: The MDH initiated mandatory reporting and submission of confirmed or possible C. auris isolates in August 2019. The MDH Public Health Laboratory (MDH-PHL) confirmed C. auris by MALDI-TOF (Bruker) from an isolate submitted by a hospital laboratory as C. duobushaemulonii to rule out C. auris. The MDH-PHL performed broth microdilution antifungal susceptibility testing (AFST). The CDC Mycotics Diseases Branch laboratory performed whole-genome sequencing (WGS). The MDH epidemiologists obtained a patient history through interviews with healthcare staff and the patient, and they collected environmental samples from otoscopes. The MDH-PHL tested environmental samples by C. auris RT-PCR and culture. The MDH recommended disinfection of examination rooms and otoscopes and 3 months of C. auris surveillance for patients evaluated with otoscopes who later returned with otic inflammation. Swabs from the patient’s axilla, groin, and external ear canals were tested for C. auris by PCR at the MDH-PHL. Results: The patient reported recurrent right ear infections in 2016 during a 16-month visit to South Korea, with treatment in multiple ENT clinics. December 2019 otitis resolved after treatment with oral amoxicillin/clavulanate and otic ciprofloxacin/dexamethasone. AFST showed resistance to fluconozale and susceptibility to 8 antifungals, including echinocandins. WGS placed the isolate in the East Asian clade, indicating similarity to isolates from South Korea and Japan. Environmental cultures were negative. The asymptomatic left ear was colonized with C. auris; other sites were negative. As of January29, 2020, no additional cases were detected. Conclusions: We identified prolonged colonization of C. auris in the external ear canals of a healthy patient. WGS and travel in South Korea, including ENT clinic exposure, provide strong evidence of C. auris acquisition in South Korea. No spread has been reported in Minnesota. Deliberate communication with clinical laboratories regarding ruling out C. auris was key to case discovery. Clinicians should be aware of C. auris epidemiology, including healthcare exposure abroad, particularly in young, healthy patients.

OBJECTIVE
To report on the prevalence and incidence of Clostridium difficile infection (CDI) from 2009 to 2013 among Veterans Healthcare Administration patients
DESIGNA retrospective descriptive analysis of data extracted from a large electronic medical record (EMR) database
SETTINGData were acquired from VHA healthcare records from 2009 to 2013 that included outpatient clinical visits, long-term care, and hospitalized care as well as pharmacy and laboratory information.
RESULTSIn 2009, there were 10,207 CDI episodes, and in 2013, there were 12,143 CDI episodes, an increase of 19.0%. The overall CDI rate increased by 8.4% from 193 episodes per 100,000 patient years in 2009 to 209 episodes per 100,000 patient years in 2013. Of the CDI episodes identified in 2009, 58% were identified during a hospitalization, and 42% were identified in an outpatient setting. In 2013, 44% of the CDI episodes were identified in an outpatient setting.
CONCLUSIONThis is one of the largest studies that has utilized timely EMR data to describe the current CDI epidemiology at the VHA. Despite an aging population with greater burden of comorbidity than the general US population, our data show that VHA CDI rates stabilized between 2011 and 2013 following increases likely attributable to the introduction of the more sensitive nucleic acid amplification tests (NAATs). The findings in this report will help establish an accurate benchmark against which both current and future VA CDI prevention initiatives can be measured.

Since the publication of “A Compendium of Strategies to Prevent Healthcare-Associated Infections in Acute Care Hospitals” in 2008, prevention of healthcare-associated infections (HAIs) has become a national priority. Despite improvements, preventable HAIs continue to occur. The 2014 updates to the Compendium were created to provide acute care hospitals with up-to-date, practical, expert guidance to assist in prioritizing and implementing their HAI prevention efforts. They are the product of a highly collaborative effort led by the Society for Healthcare Epidemiology of America (SHEA), the Infectious Diseases Society of America (IDSA), the American Hospital Association (AHA), the Association for Professionals in Infection Control and Epidemiology (APIC), and The Joint Commission, with major contributions from representatives of a number of organizations and societies with content expertise, including the Centers for Disease Control and Prevention(CDC), the Institute for Healthcare Improvement (IHI), the Pediatric Infectious Diseases Society (PIDS), the Society for Critical Care Medicine (SCCM), the Society for Hospital Medicine (SHM), and the Surgical Infection Society (SIS).

Distribution of depth to ice-cemented soils in the high-elevation Quartermain Mountains, McMurdo Dry Valleys, Antarctica

We report on 475 measurements of depth to ice-cemented ground in four high-elevation valleys of the Quartermain Mountains, McMurdo Dry Valleys, Antarctica. These valleys have pervasive ice-cemented ground, and the depth to ice-cemented ground and the ice composition may be indicators of climate change. In University Valley, the measured depth to ice-cemented ground ranges from 0–98 cm. There is an overall trend of increasing depth to ice-cemented ground with distance from a small glacier at the head of the valley, with a slope of 32 cm depth per kilometre along the valley floor. For Farnell Valley, the depth to ice-cemented ground is roughly constant (c. 30 cm) in the upper and central parts of the valley, but increases sharply as the valley descends into Beacon Valley. The two valleys north of University Valley also have extensive ice-cemented ground, with depths of 20–40 cm, but exhibit no clear patterns of ice depth with location. For all valleys there is a tendency for the variability in depth to ice-cemented ground at a site to increase with increasing depth to ice. Snow recurrence, solar insolation, and surface albedo may all be factors that cause site to site variations in these valleys.

Introduction
The 1980s were designated the International Water and Sanitation Decade, and the international community committed itself to ensuring that everyone in the world would have access to at least basic water and sanitation services by 1990. This target was not met. While hundreds of millions did receive access to new services, at the end of the decade well over 1.1 billion people still lacked improved water supplies, and more than 2.7 billion lacked sanitation services. By the year 2000, although another billion people had obtained access to improved water and sanitation services, population growth had left the number of those still unserved at roughly the same absolute level. In 2002, at the Johannesburg World Summit on Sustainable Development, the global community made a new commitment to a set of Millennium Development Goals (MDGs), including environmental sustainability. One of the targets under the environmental sustainability MDG is to cut by half the proportion of people in the world living without access to water and sanitation by 2015.
While we certainly hope that the global target for water and sanitation will be met this time, there are grounds for concern. Some important physical and economic features of water supply and sanitation make it inherently difficult to achieve broad-scale goals such as those of the International Water and Sanitation Decade and the MDG water and sanitation target – more difficult than for other MDG targets such as providing access to affordable essential drugs or communication and information technology.

Since the June 1995 ICES Symposium on Fisheries and Plankton Acoustics in Aberdeen (MacLennan and Holliday, 1996) the use of acoustics for studying zooplankton has seen important advances. Acoustical monitoring of small-scale zooplankton distributions can now be done at intervals of a fraction of a minute. Resolution at vertical spatial scales of tens of centimeters is now easily achieved with commercially available sensors. Multiple-frequency echo-ranging sensors (TAPS™) have been deployed in an up-looking mode on the bottom, and on moorings looking up, down and horizontally. Real-time telemetry provides data on plankton distributions at ranges up to tens of meters from the sensors for periods of weeks to months. These sensors allow one to estimate total zooplankton biomass and the size-abundance spectrum of the animals in the water column at different depths and times. When a profiling CTD and multi-spectral optical sensors were used to define the physical environment and phytoplankton distributions near an acoustical zooplankton profiler, bold relationships were observed between measured spatial and temporal patterns. New methods in zooplankton acoustics are illustrated with data collected from these sensors while monitoring thin, sub-meter thick layers of plankton and diel migrations of benthopelagic crustaceans.

Electron excited energy dispersive X-ray spectrometry (EDS) was used to determine the atomic species present in 35 Å diameter CdSe nanocrystallites. Both theoretical modeling and experimental calibration with standard materials were used to relate the relative X-ray intensities to the atomic fractions present in the sample. The EDS results were normalized to those found by elemental analysis, which also detects light elements. With different surface models, and the atomic fractions found by EDS, the number of surface groups per nanocrystallite was determined.
